For each lost or damaged item, fill out the appropriate form and send it to Megan at 4CLS. When the new item has been received, the 4CLS business office will send a bill to the library whose patron had the item at the time of the loss/damage. Payment is made to the business office.
If the item is no longer available, a default replacement charge of $25.00 per item will be billed to the library. If you collect the money before you’ve been billed, please send it with the bill after you receive it.
